DASH supports collaborations that seek to improve the health of their communities, promote health equity and contribute to a Culture of Health by strengthening information sharing, engaging additional sectors and building sustainable capacity. DASH aims to create a body of knowledge and advance this emerging field by identifying and sharing opportunities, barriers, lessons learned, promising practices and indicators of progress for sharing data and information across and beyond traditional health sectors. |
CCHC
Founded in 2000 by a group of committed volunteers, the Clinic offers primary care, behavioral health services, health and wellness education, preventative services, acute and chronic disease management and dental services for pediatric and adult patients. We offer outreach and enrollment services to assist people in attaining health care benefits through the Affordable Care Act |
NC MedAssist
NC MedAssist is a nonprofit pharmacy program providing access to lifesaving prescription medications, patient support, advocacy and related services to poor, vulnerable, and uninsured North Carolina residents. they seek to ensure that no eligible person in North Carolina has to choose between purchasing food or their needed medication; improving the patient’s health and lowering uncompensated health care costs for all stakeholders. |
Care Ring
Care Ring is a nonprofit organization dedicated to providing preventive health services for the uninsured, underinsured or those lacking access to affordable, high-quality preventive health care. The organization annually serves more than 7,000 people in Mecklenburg County and is a leader in collaborative efforts to improve community health. |
Institute for Social Capitol
The Institute for Social Capital, founded in 2004, became part of the larger UNC Charlotte Urban Institute in 2012. Its mission is to support university research and increase the community's capacity for data-informed decision making. At its core is a comprehensive set of administrative data gathered from governmental and nonprofit agencies in the region. By combining key sources of data into one community database, ISC provides a valuable resource to assess the impact of specific interventions across agency lines and to better understand the social and environmental variables that affect the community, particularly with regard to outcomes for children and families. |
City of Charlotte
