We use data and analytics to improve healthcare in urban regions
Overview
The journey to make Charlotte the healthiest city by 2025 must be informed, driven, and refined using data. To fully understand community health issues, data must be captured from across sectors including health and social service providers. Currently, there is no community-wide data governance framework in place that facilitates proactive data sharing across community providers. This limits the ability of organizations working on advancing public health to understand overlapping target populations and design effective programs. We want to connect this fragmented system!
Implementation
The Academy for Population Health Innovation (APHI) will lead implementation of this initiative and ensure that data is evaluated, integrated, and managed with both our high-security standards and project goals in mind. APHI is a partnership between the University of North Carolina at Charlotte (UNCC) and Mecklenburg County Public Health that focuses on the application of data to understand and address community health needs, including health disparities. Central to APHI’s mission are cross-sector partnerships and data sharing agreements established with individual community entities.
National level support will be provided by the Data Across Sectors for Health (DASH) and National Interoperability Council (NIC).

The Community DRIVE project is aimed at improving the ability of health care providers to help and heal the communities they serve through high quality data evaluation and integration.
We will increase communication between community partners and decrease the latency between when data is collected and when it is used.

DRIVE Community Health Forward
Health can not be defined or measured as a single attribute. It is a combination of multiple variables combined to achieve a holistic view of risk. The goal of Community DRIVE is to build an integrated health platform in which all members of the local Health Community can access and share data for the benefit of their neighbors.
